How to Verify Insurance
Please call us at 480-429-9400 to verify your insurance coverage. Be aware that benefits can only be verified Monday through Friday. Depending on your insurer, it may take 24 hours for verification after you provide your info but please do not get discouraged! We know this is an important step for you and we will be in touch with an answer as soon as possible.
Below is information you should have available when you call.
Information about the patient:
- Full Name
- Date of Birth
- Social Security Number
- Email Address
- Residence Address
- Name of Employer (if employed)
- Primary Phone
Information about insurance:
- Insurance Provider Name
- Insurance Member ID
- Insurance Group ID
- Type of Plan (We do NOT accept HMO, Medicare or AHCCCS.)
- Insurance Provider Phone
If patient is different from policy holder:
- Policy Holder’s Full Name
- Last 4 of Social Security
- Date of Birth
For your initial appointment, PLEASE ARRIVE 30 MINUTES EARLY and be sure to bring your insurance card. A $200 non-refundable deposit is required for initial evaluation appointment which, based on subsequent payment by your insurer, may be adjusted and become a credit on account.
